Doctor’s Agreement to Support the Foundation for Eye Health Awareness
I pledge to support The Foundation for Eye Health Awareness and its mission to educate Americans about the importance of eye care and the value of a comprehensive eye examination.
In order to advance the efforts of the Foundation, I will:
- Ensure that I or my staff discusses the health and wellness factors of the eye exam with every patient
- Confirm that each patient knows when to return for their next eye exam and why, and how my office will help them remember when it is time for the next eye exam
- Discuss with each patient what eye exam schedule is appropriate for family members
- Support other professionals and the companies that support the Foundation by prescribing or recommending their products and services, when those products and services meet the needs of my patients and such actions are consistent with my professional oath and responsibilities
- Ask each of my vendors and suppliers to support the Foundation with a financial contribution that is proportional to their size, and by pledging to reinforce the messages of the Foundation, including the Think About Your Eyes campaign
- Display Think About Your Eyes consumer education materials and signs in my offices
- Make sure that my location information on the Think About Your Eyes website eye doctor locator is accurate and kept current
Optician’s Agreement to Support the Foundation for Eye Health Awareness
- Ensure that all of my clients have discussed the following items with their eye doctor:
- the health and wellness factors of an eye exam and what eye exam schedule is appropriate for family members.
- when to return for their next eye exam and why, and how my office (if possible) will help them remember when it is time for the next eye exam
- Support other professionals and the companies that support the Foundation by recommending their products and services, when those products and services meet the needs of my patients and such actions are consistent with my professional oath and responsibilities
- Ask each of my vendors and suppliers to support the Foundation with a financial contribution that is proportional to their size, and by pledging to reinforce the messages of the Foundation, including the Think About Your Eyes campaign
- Display Think About Your Eyes consumer education materials and signs in my offices
